Assessments

Assessments are completed over the phone and usually take about 20 minutes. You will be asked for information including:

  • name;
  • address;
  • phone number;
  • next of kin;
  • local doctor;
  • medical conditions;
  • disabilities, if any; and
  • ability to access transport.

The information collected will be noted on your Service User record.

You will also be asked for permission to provide some of your information to government departments in accordance with our Funding Body agreements, such as Ageing, Disability and Home Care, but your NAME and ADDRESS will not be included (see Service User Privacy and Confidentiality Policy). By providing such information you will be assisting in the planning of future services and funding for Community groups in your area. You have the right to refuse your information being given to a government department.

After registration with the Service, we will send you a Service User Information Handbook which provides information about the services offered and your rights as a Service User.

If you are assessed as not being eligible to use our service, we will suggest other services which may be more appropriate, and you will be advised of circumstances in which you could reapply.
